The division in which your child will play is
determined by the year in which he/she was born. Please refer to the following chart for divisions
and 2012 registration fees.

Registration price includes a pair of SMLA shorts and t-shirt (issued through team managers).

Intermediate registration is done at try-outs.

Returning players will receive priority registration only
until after in-person registration is complete February 4. After
such time, registration will be on a first come, first served basis. SMLA
reserves the right to wait-list late registrants after initial team numbers have
been reached in an effort to create another full team for play in the same
division.

SMLA uses both in-person and online registrations, as follows:

Registrations will be received as follows:

All registrations must be completed online including those done in-person.

If unable to access the online registration portal, please contact the SMLA
Registrar (registrar@semiahmoolacrosse.com).

Payments can be made by credit card (preferable) or by cheque.

After completing the online registration, each family must mail in a Volunteer
Cheque for $125.00 post-dated for June 1, 2012 with your child’s name clearly marked in the memo area.

All families paying by cheque must also mail-in a Registration Cheque for the fees allotted to your child’s division (one cheque per child with each child’s name clearly marked in the memo area of the co-ordinating cheque).

Please Note: No cash will be accepted!

For new players only:
-
Provide a photocopy of each registering player’s birth certificate. The BCLA requires each association to maintain ’proof of age’ records.
-
Provide ’proof of residency’. A photocopy of a recent utility bill (within 3 months) showing both your current address and the family name is sufficient.

In-Person Registration
When registering in-person, please ensure you bring the following:

One registration fee cheque per player, NO CASH (e.g. if registering 2 players, provide 2 separate cheques). Please refer to the chart above for current registration fees.

A $125.00 ’Volunteer’ cheque, post-dated to June 1, 2012 (1 per FAMILY). Please see information below regarding SMLA’s volunteer program.

Each player’s BC medical card - you will need to fill out the BC medical number on the registration form (new & returning players).

For new players only:

Semiahmoo Lacrosse runs on volunteers. A $125.00 volunteer fee will apply per family for those who do not volunteer. A post-dated cheque will be collected at registration.

All refunds are subject to a $25.00 administration fee.

Cancellations prior to March 1st will receive a full refund, less the $25.00 administration fee.

Cancellations between March 2nd and April 15th will receive a 50% refund, less the $25.00 administration fee.

Cancellations after April 15th will not be given a refund.
To claim a refund, please complete and submit the
Refund Form to the SMLA Treasurer (treasurer@semiahmoolacrosse.com).

The KidSport™ Fund is
a program of Sport BC designed to "remove
the financial obstacles that prevent
underprivileged children from playing sport
in British Columbia". Each child aged
6 - 18 who applies is eligible for up to
$200 for one season of sport. The
grant is provided directly to the club or
sport organization. For information on

The objective of Let
Kids Play Foundation is to increase the
health, enhance the education and develop
the skills of low-income and underprivileged
youth by providing funds to enable them to
participate in organized sporting
activities. Each child aged 17 and
under who applies is eligible for up to $200
for one season of sport. For
